Basic Description:
A new county-wide program that provides three home visits over the course of the first year of a baby's life. Visits will be family focused and parent-driven, providing information on things like nutrition, childcare, health & safety, child development, & parent support programs. It is designed to be a resource and referral program.
ELI has teamed up with St. Joseph's Home Care in order to serve the 390 families during the first year of this new program. St. Joseph's will do the first visit, to focus on the needs of the family and baby as they come home from the hospital. ELI will do the second and third home visit, focusing more on development and parenting issues.
Who is served/How do they qualify:
All first time families in Sonoma County are eligible. Our agencies will serve 390 of those families, in various locations.
Who works in this Program:
A variety of specialists from the Early Intervention team. Oversight is provided by the Program Manager and ultimately, the Clinical Director.
